Online Accident/Injury Submission Guidelines

Guidelines for submitting online accident/injury reports.

Medic Report Form

This form must be uploaded with the corresponding online Accident/Injury Report.

Vet Report Form

This form must be uploaded with the corresponding online Accident/Illness Report.

USEF Refusal of Evaluation

Refusal of evaluation by the qualified medical personnel at a competition.

Return to Competition Medical Release

This form is to be used by Federation athletes who have been assessed to have a possible concussion or apparent head injury. This form must be completed and returned to the Federation per rule GR848.6 to be eligible to return to competition.
